Authorization and liability release

1. Authorization to work

I authorize Husky Dad Roadside Rescue LLC (the Company) and its technicians to perform the roadside service quoted to me by text, including lockout entry, tire changes, battery jump starts and related work, on the vehicle described below. I confirm that I am the owner of the vehicle or am authorized by the owner to approve this work.

2. Quote and payment

I have agreed to the quote provided by text before dispatch. I agree to pay the full agreed amount when the service is complete, by cash or by card through Square tap to pay. Additional work will be performed only with my approval.

3. Known risks

I understand that roadside work carries risk. Lockout tools can scratch, bend or wear weather stripping, door frames, paint or window components. Jump starts can affect vehicle electronics, and a battery that is failing, frozen or damaged may not respond. Tire changes can reveal rusted, seized, stripped or damaged wheel hardware, and removing a wheel may expose worn parts.

4. Existing condition of the vehicle

I acknowledge that the Company is not responsible for damage, defects or wear that existed before service, or for any problem that is discovered or becomes apparent because of the vehicle's age, condition or prior repairs. I understand the Company is not providing a mechanical diagnosis or repair beyond the service quoted.

5. Release and limitation of liability

To the fullest extent permitted by law, I release and hold harmless the Company, its owner, employees and agents from claims, damages, losses and expenses arising from the authorized work, except those caused by their gross negligence or willful misconduct. The Company's total liability, if any, will not exceed the amount I paid for the service. The Company is not liable for lost time, towing, missed appointments or other indirect or consequential losses.

6. Safety and personal property

I will remain in a safe location away from traffic while work is performed and will remove pets and valuables from the vehicle where possible. The Company is not responsible for personal property left in or on the vehicle.

7. General terms

This authorization is governed by the laws of the State of Colorado. If any part of it is found unenforceable, the remaining parts stay in effect. My electronic signature below has the same effect as a handwritten signature.
